RULES The Mindwave Online Advisory Forum Membership Profile Drawing Rules (These rules and drawing apply only to the Sign-up profiling survey. There are individual drawings for participation in each survey after joining this forum. To view rules for other survey(s), please refer to the unique rules link specified for that survey.) No purchase necessary to enter. The promotion for this survey begins on the first day of each quarter (January 1, April 1, July 1, and October 1) and ends on the last day of each quarter (March 31, June 30, September 30, and December 31). The drawing will be held approximately 2 weeks after the end of each quarter (April 15, July 15, October 15, and January 15) Prizes: To be decided Odds: Odds of winning will be determined by the number of entries received. Entry/participation: To be officially entered in the drawing, complete The Mindwave Online Advisory Forum Profile survey. Only one entry per person is allowed. Completion of the survey is not necessary to be entered into the drawing. You may also enter the drawing by mailing a 3" x 5" card containing your name, address, email address, and telephone number to: Mindwave Research, Inc. Attn: Mindwave Online Advisory Forum Membership Profile Quarterly Drawing 511 W. 7th St. Austin, Texas 78701. Mail in entries must be postmarked by the last day of each quarter (March 31, June 30, September 30, and December 31) and received no later than 2 weeks after the end of each quarter (April 15, July 15, October 15, and January 15). Hand delivered entries will not be accepted. Mindwave Research, Inc. is not responsible for mail entries that are late, lost, misdirected, mutilated, or illegible or those with insufficient postage. Selection of winner: Winners will be randomly selected approximately 2 weeks after the end of each quarter (April 15, July 15, October 15, and January 15). Mindwave Inc. and Mindwave Research, Inc. reserve the right to modify scheduling of the contest without prior notice. Limit one prize per person. Prize winners will be notified approximately 4 weeks after the end of each quarter (April 29, July 29, October 29, and January 29) via email to the address submitted in the entry. In order to claim your prize, you must reply by email within 14 days or another winner will be randomly selected. Winners must execute an affidavit of eligibility in order to receive the prize. Neither Mindwave, Inc. nor Mindwave Research, Inc. is responsible for notifications that are misdirected or not received because of addresses that are no longer correct. Any prize notification or prize returned as undeliverable will result in selection of an alternate winner who will be notified by email and given 14 days to respond. Redemption: Mindwave Research, Inc. will deliver the prize in the form of a certified check made payable to the entrant, at the address supplied in the email to response to the notification of winning the prize. Eligibility: All legal US residents 18 years of age or older. Canadian entrants - All legal Canadian residents (except residents of the province of Quebec) who have reached the age of majority. Void where prohibited or restricted by law. Employees of Mindwave Research, Inc. and their immediate family(s) or household members are not eligible. Taxes, and additional fees in excess of the prize amount, are the sole responsibility of the prize winner. Unaccepted prizes may not be awarded. Prize is non-transferable. Winner is subject to these complete and official rules. Mindwave Research, Inc. is not in any way liable for damage, loss, or injury resulting from computer malfunctions, misdirected or incomplete entries. Winners assume liability for injuries, damages, losses or expenses of any kind caused or claimed to be caused by participation, or by the acceptance, possession, use or misuse of any prizes. By participating and redeeming a prize, winner releases Mindwave Inc. and Mindwave Research, Inc., their directors, officers, employees and agents, from any and all loss, liability or damages resulting from or arising out of participation in this contest. Winner List: The name of the prize winners may be obtained approximately 6 weeks after the end of each quarter (May 15, August 15, November 15, and February 15) by sending a self addressed stamped envelope with a written request indicating "please send me the prize winner list for the "The Mindwave Online Advisory Forum Membership Profile Quarterly Drawing" (WA and VT residents may omit return postage, as may residents of any other state in which this is a legal requirement) to: Mindwave Research, Inc.
